Home Health Physical Therapist
A Home Health Physical Therapist is needed in Brooklyn, NY.

Salary and Benefits
- Salary range: $110,000 – $130,000 annually
- 25 PTO days in year one
- 6 paid holidays

Responsibilities
- Provide in-home physical therapy following the physician’s care plan
- Achieve 30 points per week, treating 6 patients per day
- Assess, treat, and adjust individualized care plans to improve mobility and recovery
- Monitor patient progress, document outcomes, and adjust treatment as needed
- Educate family members and caregivers on rehabilitation techniques and patient care
- Collaborate with the care team to ensure comprehensive patient care
- OASIS experience preferred
Requirements
- Current Physical Therapy Licensure in New York State
- Graduate of an accredited PT program
- Proven experience in home health or physical therapy
- Strong communication and interpersonal skills
- Knowledge of current treatment practices and clinical expertise
