We’re hiring a Home Health Physical Therapist (PT) who wants schedule autonomy, one-on-one patient care, and the ability to increase earning potential through productivity.

You’ll step into a home health role serving North Jacksonville. You’ll manage your own schedule while providing evaluations, starts of care, reassessments, discharges, and ongoing physical therapy services in patients’ homes.

Compensation & Benefits

• Estimated annual earnings: $85,000–$120,000 based on productivity and experience
• Health insurance: Employer pays 50%

• PTO: 10 days during Year 1, increasing to 11 days during Year 2
