The Physical Therapist will provide in-home therapy services, assess patient functional levels, and develop customized treatment plans to improve mobility, independence, and cognitive function. Responsibilities also include patient and caregiver education, supervision, and progress documentation.
Key Responsibilities
Perform Physical Therapy assessments and develop treatment programs
Record and report patient's reaction to treatment and condition changes
Instruct patients and caregivers in therapy programs and device use
Supervise physical therapy assistant and, when appropriate, the certified nursing assistant
Prepare reports and participate in staff and Agency development activities

We are looking for compassionate physical therapists with:
Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college required
Licensed Physical Therapist issued by the state in which you will be working.
Minimum of one year healthcare experience
Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
A commitment to consistently meet critical deadlines for charting
The skills needed to self-manage your time and schedule
Demonstrated experience with tablets, mobile phones and EMR software
PPV position-Rates $60+ depending on type of visit
