    Verona, PA

    As a Physical Therapist in our client's Home Health team, you will be responsible for providing Physical Therapy services to patients in their homes, assessing patient functioning levels, and developing treatment programs to improve independence and physical/cognitive functioning. You will also be involved in patient and caregiver education, supervision, and reporting on patient progress. We are open to new graduates for this position. We are also open to part time or PRN candidates as well. 

    Key Responsibilities

    Perform Physical Therapy assessments and develop treatment programs

    Record and report patient's reaction to treatment and condition changes

    Instruct patients and caregivers in therapy programs and device use

    Supervise physical therapy assistant and, when appropriate, the certified nursing assistant

    Prepare reports and participate in staff and Agency development activities

    We are looking for compassionate physical therapists with:

    Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college required

    Licensed Physical Therapist issued by the state in which you will be working.

    Minimum of one year healthcare experience but open to new graduates

    Current driver’s license and ability to spend ~20% of your day driving to/from patient locations

    A commitment to consistently meet critical deadlines for charting

    The skills needed to self-manage your time and schedule

    Demonstrated experience with tablets, mobile phones and EMR software
