About the Role:

Join Bellestar Home Health Care Inc as a Home Health RN and make a difference in the lives of patients throughout various counties. We are seeking compassionate and dedicated registered nurses to provide exceptional in-home care and support to our clients.

Responsibilities:

Conduct comprehensive patient assessments and develop individualized care plans.

Administer medications and monitor patient responses to treatments.

Provide wound care and manage medical equipment as needed.

Educate patients and families on health management and wellness strategies.

Collaborate with interdisciplinary teams to ensure quality care delivery.

Document patient progress and maintain accurate medical records.

Assist with activities of daily living and promote patient independence.

Participate in ongoing training and professional development opportunities.

Requirements:

Current RN license in the state of California.

At least 1 year of nursing experience, preferably in home health care.

Strong clinical assessment and critical thinking skills.

Excellent communication and interpersonal abilities.

Ability to work independently and manage time effectively.

Compassionate demeanor and commitment to patient-centered care.

CPR and First Aid certification required.

Reliable transportation and valid driver's license.
