Clinical Case Manager – Home Health (RN)
Position Overview
We are seeking an experienced Clinical Case Manager (RN) to join our Home Health office team. This in-office position is responsible for overseeing clinical case management, reviewing OASIS documentation, ensuring regulatory compliance, coordinating patient care, and supporting the delivery of high-quality home health services. The ideal candidate is highly organized, detail-oriented, and experienced in Medicare-certified home health operations.

Requirements
- Active, unrestricted Registered Nurse (RN) license in the State of Florida
- Minimum of 3 years of Home Health experience
- Strong knowledge and experience with OASIS
- Working knowledge of Medicare Home Health Guidelines
- Experience obtaining and managing authorizations
- Proficient in Microsoft Office (Word, Excel, Outlook, and PowerPoint)
- Strong computer and documentation skills
- Excellent communication, organizational, and time management abilities
Preferred Qualifications
- Previous experience in Clinical Case Management
- Experience working in a Medicare-certified Home Health Agency
- Knowledge of physician order management and clinical documentation compliance
- Experience coordinating interdisciplinary patient care
- Familiarity with EMR systems used in Home Health
Responsibilities
- Review RN OASIS assessments for completeness, accuracy, and compliance
- Manage patient cases from admission through discharge
- Assist with developing and reviewing patient service plans
- Coordinate care among physicians, clinicians, and interdisciplinary team members
- Ensure patient visits are scheduled and assigned appropriately
- Monitor patient progress and follow up on ongoing cases
- Ensure timely initiation of care for all patients
- Review clinical records to ensure services are provided according to the Plan of Care (POC)
- Track and obtain signed physician orders
- Communicate with physicians and insurance providers regarding changes in patient condition and updated orders
- Conduct and document interdisciplinary case conferences
- Verify clinical documentation for billing and reimbursement
- Ensure complete and accurate patient records in accordance with Medicare regulations
- Build and maintain strong working relationships with clinical and administrative staff
- Support quality assurance initiatives and regulatory compliance throughout the case management process
