Sign-On Bonus

Looking for a compassionate and skilled nurse to join our team. You'll play a key role in providing comprehensive care for our patients, from admission to end-of-life.

Your responsibilities will include:

• Conducting initial and ongoing assessments of patients' physical, functional, and psychosocial needs in their place of residence.

• Developing and implementing personalized care plans to ensure quality and continuity of care.

• Educating patients and their families on disease processes, self-care, and end-of-life care.

• Ensuring patients meet the criteria for home health/hospice admission and re-certification.

• Providing clinical direction to home health aides and LPNs/LVNs.

• Completing all necessary documentation, including OASIS assessments and progress notes, to track patient progress and ensure a high standard of care.

• Communicating regularly with physicians and the interdisciplinary team about patient needs and changes.

Qualifications

EXPERIENCE AND REQUIRED SKILLS

Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.

• Licensure : Current state license as a registered nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact. Current driver’s license.
• Experience : Experience as a RN in a Home Health Setting. Hospice experience is a bonus.
• Skills : Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Specialized skills preferred in palliation and end-of-life. Must read, write and comprehend English.

Additional Information

$90,000 year Salary plus $10,000 Sign-On Bonus
