Sign-On Bonus
Looking for a compassionate and skilled nurse to join our team. You'll play a key role in providing comprehensive care for our patients, from admission to end-of-life.
Your responsibilities will include:
Conducting initial and ongoing assessments of patients' physical, functional, and psychosocial needs in their place of residence.
Developing and implementing personalized care plans to ensure quality and continuity of care.
Educating patients and their families on disease processes, self-care, and end-of-life care.
Ensuring patients meet the criteria for home health/hospice admission and re-certification.
Providing clinical direction to home health aides and LPNs/LVNs.
Completing all necessary documentation, including OASIS assessments and progress notes, to track patient progress and ensure a high standard of care.
Communicating regularly with physicians and the interdisciplinary team about patient needs and changes.
Qualifications
EXPERIENCE AND REQUIRED SKILLS
Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Additional Information
$90,000 year Salary plus $10,000 Sign-On Bonus
