    What You’ll Do

    • Lead in-home patient assessments and develop individualized care plans

    • Guide patients and families through treatment, education, and medications

    • Collaborate with care teams and physicians to ensure seamless coordination

    • Document visits and patient progress using your OASIS expertise

    • Bring trust, empathy, and professionalism into every home you enter

    What You Bring

    • Active Arizona RN license and current CPR/BLS certification

    • Experience in home health nursing and strong OASIS documentation skills

    • Reliable transportation, valid driver’s license, and clean background check

    • TB test and fingerprint clearance (we’ll help you get these if needed)

    • Bilingual (especially Spanish-speaking) is a plus

    • RN degree from an accredited nursing program
