What You’ll Do:

• Perform in-home assessments and develop individualized care plans
• Guide patients and families through treatment and medications
• Document progress with precision using your OASIS expertise
• Collaborate with care teams and physicians for smooth, effective care
• Be a trusted presence in the homes and lives of your patients

What You Bring:

• Active AZ RN license and current CPR/BLS certification
• Experience in home health and OASIS documentation
• Reliable transportation, valid driver’s license, clean background
• TB test and AZ fingerprint clearance (we can help if needed)
• Bilingual (especially Spanish-speaking) is a plus
• RN degree from an accredited nursing program
