Able Home Healthcare is looking for a compassionate and skilled Home Health Registered Nurse to join our growing team in San Diego, CA. Responsibilities:

Conduct comprehensive in-home patient assessments and develop individualized care plans

Administer medications, wound care, IV therapy, and other skilled nursing treatments

Monitor and document patient progress, vital signs, and changes in condition

Coordinate care with physicians, therapists, and other members of the interdisciplinary team

Educate patients and family members on disease management, medications, and home safety

Ensure compliance with Medicare, Medicaid, and home health regulatory standards

Maintain accurate and timely clinical documentation using electronic health records (EHR)

Requirements:

Active California Registered Nurse (RN) license in good standing

Minimum 1-2 years of clinical nursing experience; home health experience preferred

Strong knowledge of Medicare home health guidelines and OASIS documentation

Valid California driver's license, reliable transportation, and current auto insurance

Excellent clinical assessment, critical thinking, and time management skills

Proficiency with electronic health records (EHR) and point-of-care documentation

CPR/BLS certification required

Compassionate, self-motivated, and comfortable working independently in the field
