Registered Nurse – Home Health

We are working with a progressive, well-run, financially stable home health organization serving Pasco County. This team is committed to providing high-quality patient care.

Compensation & Benefits

• $90,000–$100,000 annual salary

Schedule

• Monday–Friday, 8:30 AM–5:00 PM
• On-call rotation: 1 week per month

Responsibilities

• Provide skilled nursing care in patients’ homes
• Complete OASIS assessments and develop individualized care plans
• Administer medications and treatments
• Monitor patient progress and adjust care as needed
• Coordinate care with the healthcare team
• Educate patients and families
• Perform diagnostic procedures and nursing interventions
• Support discharge planning and transitions of care

Requirements & Qualifications

• Active Florida RN license
• At least 2 years of RN experience, including 1 year in home health or a related field
• Strong case management and care coordination skills
• Solid clinical assessment, care planning, communication, and judgment skills
• Experience in hospital or emergency department settings and discharge planning
• Behavioral health or utilization management experience preferred
• BSN preferred
• CPR certification required; online certification not accepted
• Valid driver’s license and auto insurance
• Required CEUs in elder abuse, human trafficking, and Alzheimer’s/dementia
• Current TB test
