What You'll Do
- Assess each new patient's condition, history, and home care needs
- Build the care plan with the patient and family
- Update the care plan as the patient's status changes
- Administer medications and treatments as prescribed
- Teach patients and families how to manage care at home
- Coordinate with physicians and send updates at least every 60 days
- Coordinate with community health partners involved in the patient's care
- Plan and carry out discharge when the patient is ready
- Arrange any equipment or services the patient needs
- Check in on home health aide care every two weeks
Qualifications
- Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting.
- Current licensure in state and CPR certification.
- Preferred: Bachelor's degree, with one (1) year of home health care experience.
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
Physical Demands
This role requires the following, with or without reasonable accommodation:
- Prolonged walking and standing
- Lifting and carrying up to 25 lbs frequently and up to 50 lbs occasionally
- Lifting, positioning, and transferring patients (with assistance for patients over 50 lbs, per agency safe-handling policy)
- Reaching, stooping, bending, kneeling, and crouching
- Visual acuity and hearing sufficient to perform nursing duties
Health Requirements
Required before providing patient care:
- Pre-employment physical screening, or a letter from a recent healthcare encounter confirming the candidate is free of communicable disease
- Documented negative TB test (TST or TB blood test) taken within the past 12 months
- TB risk assessment and symptom evaluation upon hire
