A Home Health Registered Nurse is needed in Brooklyn, NY.

Salary and Benefits
- Salary range: $100,000 – $115,000 annually
- 25 PTO days in year one
- 6 paid holidays

Responsibilities
- Setting: Home Health
- Patient volume: 6–7 patients per day
- 33 productivity points required weekly
- Admission – 2 points
- Recertification/Resumption of Care – 1.25 points
- Routine Visit – 1 point
- Infusion Admission – 2.5 points
- Infusion Revisit – 1.5 points
- Schedule: Monday through Friday, 9:00am to 5:00pm
- Weekend requirement: Two weekend days per month (paid fee-for-service)
- No on-call requirements
- EMR: Homecare Homebase (HCHB)
- Documentation: OASIS experience preferred
Requirements
- Active Registered Nurse (RN) license in the state of New York
- Associate or Bachelor’s degree in Nursing
- Minimum of one year of clinical nursing experience preferred
- OASIS experience strongly preferred
- Previous home health experience is a plus
