Home Health Medical Social Worker Wanted! We're looking for a compassionate and driven Home Health Medical Social Worker to join our team!

Your responsibility is to assess and address the social and emotional needs of our patients and their families.

You'll work alongside other healthcare professionals to develop and implement care plans that support patients' well-being and recovery

To qualify, you must hold a Master's degree in Social Work and have a valid Medical Social Worker license in the state.

Experience in home health or hospice is preferred.

Strong communication skills, problem-solving abilities, and a passion for helping others are a must.

Job Functions

Assesses the psychosocial status of patients related to the patient's illness and environment.

Carries out social evaluations and plans intervention based on evaluation findings.

Maintains clinical records on all patients referred to social work.

Provides information and referral services for Organization patients and families/caregivers regarding practical and environmental needs.

Preferences

Graduate of a Master's of Social Work

Minimum of one year's experience in health care.

Experience in a home health care preferred.

Pay: From $100.00 per hour
