We are seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our Home Health team on a part-time basis. This role offers flexibility, autonomy, and the opportunity to provide meaningful one-on-one care in the home environment.

Position Details:

Responsibilities:

• Conduct speech, language, cognitive, and swallowing evaluations
• Develop individualized treatment plans
• Provide therapy for speech, language, voice, fluency, and dysphagia disorders
• Educate patients and caregivers on therapy goals and home strategies
• Collaborate with physicians and interdisciplinary team members
• Complete accurate and timely documentation
• Ensure compliance with Medicare and home health regulations

Qualifications:

• Active Speech-Language Pathologist (SLP) license
• Certificate of Clinical Competence (CCC-SLP) preferred
• Home Health experience preferred but not required
• Strong clinical assessment and documentation skills
• Valid driver’s license and reliable transportation
• Ability to work independently
