Speech Therapist (ST/SLP) – Home Health

Health Care 4 All, Inc. is seeking a compassionate and experienced Speech-Language Pathologist (SLP) to provide quality, patient-centered care in the home setting.

Responsibilities

Evaluate patients and develop individualized treatment plans.

Provide skilled speech therapy services according to the plan of care.

Establish and monitor short- and long-term goals.

Document patient care, progress, and outcomes accurately.

Educate patients and caregivers on treatment and home programs.

Collaborate with physicians and other healthcare professionals.

Qualifications

Active Florida Speech-Language Pathologist license.

Master's degree in Speech-Language Pathology.

Strong communication and organizational skills.

Excellent documentation and charting skills.

Ability to work independently.

Home health experience preferred.
