Home Health and Hospice Case Manager (RN)

A Home Health and Hospice Case Manager is needed in Katy, TX.

Salary and Benefits

• The pay range for this position is $80,000 – $90,000+
• Health insurance ($250 reimbursement per pay period)
• Vision and dental insurance

• 10 days of PTO (increasing annually with tenure)

Responsibilities

• Monday – Friday schedule
• Patient census: 20+ and growing
• EMR: Kinnser / WellSky
• Manage a caseload of home health and hospice patients
• Complete and coordinate OASIS assessments and documentation
• Organize admissions, plan of care, and patient care procedures
• Collaborate with interdisciplinary team members to ensure quality outcomes
• Provide direct nursing care and case management services to patients and families

Requirements

• Clear and active Texas RN license
• Medicare home health and/or hospice experience
• OASIS knowledge
• Strong clinical and organizational skills
