RN Home Health Case Manager- Mon-Fri Day Shift - $40-$45/HR Wage Based on Qualifications and Experience- Health, Vision and Dental Benefits

Home Helpers Home Care of Manassas is rapidly expanding. You will be responsible for performing client care assessments, caregiver training, and occasional supervision of Home Health Aides and CNAs.

Qualifications: ● Must have a state or compact nursing license ● At least 3-5 years of clinical experience ● Adaptable to working with individuals in the home ● Excellent communication and time-management skills ● Ability to travel to see multiple individuals per day ● Good teaching and training qualities

Responsibilities: ● Home Health position that requires a nurse with astute assessment skills ● You will be responsible for performing client care assessments, caregiver training, and occasional supervision of Home Health Aides and CNAs ● Perform initial and follow-up care assessments on individuals in their homes. ● Perform vital signs and medication reviews. ● Assess individuals for skin integrity and make recommendations. ● Initiates and documents a plan of care for the client based on the established goals from the care assessment and discussion with the family. ● Be available for caregiver and client concerns and requests, as needed
