Position Overview
We’re seeking a Registered Nurse (RN) to join our growing team. This role combines clinical excellence with personal connection — providing in-home assessments, developing client care plans, and supporting caregivers to deliver safe, person-centered care.
The RN also plays an important role in maintaining compliance with state regulations and building strong relationships with clients, families, and community partners. You’ll work closely with our care operations team to ensure every client receives attentive, high-quality service.
Key Responsibilities
Conduct in-home visits to assess new clients and develop individualized care plans
Complete supervisory visits and update care plans as needed
Support and educate caregivers to ensure excellent care and proper documentation
Deliver annual in-service trainings and track required caregiver certifications
Maintain accurate records of assessments, visits, and communications
Collaborate with the care operations team to support scheduling and follow-up
Communicate effectively with clients, families, and referral sources
Participate in outreach or marketing events to strengthen community relationships
Qualifications
Active RN license in good standing with the South Carolina Board of Nursing
Minimum 2 years of nursing experience (home care or community health preferred)
Strong clinical judgment and communication skills
Compassionate, reliable, and organized
BLS Certification required
Reliable transportation and ability to travel locally
