    HomeCare.com is currently hiring multiple Licensed Practical Nurses (LPNs) for PRN/Per Diem work at Skilled Nursing and Rehabilitation facilities in the Philadelphia area. We offer tons of flexibility in your schedule, with 8- or 12-hour shifts, days or nights, weekdays or weekends.

    We are offering starting hourly rates of $26/hour and will soon be adding comprehensive benefits packages for LPNs working a minimum number of shifts per month. Opportunities for long-term employment may be available at the end of contract.

    ABOUT THIS POSITION:

    The Licensed Practical Nurse (LPN) assists with nursing assessments and care planning, administers direct resident care, and supervises resident care provided by CNAs on the nursing team.

    Licensed Practical Nurse (LPN) Responsibilities:

    Provides quality nursing care to residents to maintain or attain the highest practical level of functioning

    Assesses residents for any decline of condition, signs of change in condition, weight loss, decline in ambulation, etc.

    Notifies appropriate staff per facility protocol of the need for evaluation and informs staff on any changes in assessment

    Administers medications and treatments to residents which includes monitoring administration of intravenous fluids and placing orders for medication

    Confer with residents and families on any issues relating to patient care and services

    Make referrals to other departments when appropriate

    Create employee and resident incident reports and complete follow-up investigation forms

    Maintain accountability for controlled substances by reviewing inventory medications with another licensed nurse and notifying supervisors of discrepancies

    Supervise CNAs and unit clerk in regards to work procedures, expediting work flow, evaluations, counseling, etc.

    Licensed Practical Nurse (LPN) Requirements:

    Must have an active license as a Licensed Practical Nurse (LPN) in the State of Pennsylvania

    Graduate from an accredited School of Nursing

    Team player, strategic thinker and quick to get up to speed
