Summary/Objective Through CAN, the Homeless Street Outreach Program (HSO), will support and work with street homeless individuals to help them find shelter, stable housing, resources, and services. The Street Homeless Outreach Team will provide responsive intensive outreach services to individuals and families who are street homeless by focusing on developing meaningful partnerships with the goal of connecting them to essential services which will lead to housing.
Essential Functions:
Approximately 90% of the time will be allocated in the community, directly providing services to people who are street homeless. Candidates must be comfortable working in diverse settings, including shelters, streets, jails, hospitals, encampments, etc. While a significant amount of the work is independent and self-directed, there is also a dedicated team dynamic.
Determine overall needs of those in campsites and other places not meant for habitation to strategically manage time and resources. Proactively initiate non-judgmental conversation and a consistent presence; offer clothing, hygiene articles, and other basic items.
The ability to respect individual choice and a commitment to serving homeless populations is required. Also, this position requires excellent verbal and written communication skills, the ability to manage a caseload and the understanding to respect the abilities and challenges of people served.
Provide intensive quality and consistent outreach services to individuals on the streets. Work in a person-centered environment. Link, advocate and support people in obtaining needed services and resources, including linkages to follow up case management.
Provide the Team Leader with client information in an accurate and timely manner. Ensure fiscal compliance with agency equipment, vehicles, and funds. Ensure client records are complete and in compliance with all required standards.
Participate in supervisory sessions weekly and/or situational supervisory sessions with the Program Director to review and plan the weekly schedule, review note corrections and deal with other issues as needed.
Perform other related work as directed i.e., documentation in Homeless Information Management System (HMIS), administrative work, scheduling, regular communication with team members.

Ability to communicate effectively both verbally and in writing.
Lived experience of homelessness preferred.
HS Diploma/GED required; college degree in social sciences preferred. At least one year’s previous work experience in mental health programs and experience working with others.
