    Community CARES serves and strengthens the community by providing emergency shelter, resources, and supportive services to individuals and families at risk of or experiencing homelessness by providing shelter, outreach, and housing-focused services to individuals and families in Cumberland County, PA. Through a trauma-informed, person-centered approach, we aim to stabilize housing, restore dignity, and reduce barriers that prevent long-term housing success. The Homelessness Prevention and Diversion Case Worker is responsible for assisting individuals and families in resolving their immediate housing crisis before entering shelter by facilitating creative and sustainable alternatives. The role diversion support, Coordinated Entry access, and housing counseling services compliant with HUD standards. This position is essential to Community CARES’ strategy to reduce inflow into homelessness, especially among underserved and disproportionately impacted populations.

    This position supports clients at the front door of the homeless system, helping them identify viable solutions to stay housed or secure alternative housing outside of emergency shelter.

    Prevention, Diversion, and Case Management

    Provide housing-focused assessments to determine immediate threats to housing stability or reasons for housing loss.

    Use diversion practices to help clients identify non-shelter alternatives, such as staying with family/friends, conflict resolution, relocation, or short-term financial support.

    Develop personalized Diversion and Housing Stability Plans with clients that explore all safe, available options to avoid homelessness.

    Offer financial assistance and in-kind support (e.g., rental assistance, transportation, mediation, basic needs) to prevent loss of housing.

    Support lease education, budgeting, and referrals to mainstream services (e.g., utilities, legal aid, employment).

    Provide follow-up case management to support ongoing stability and reduce recurrence of housing crises.

    Coordinated Entry and CoC Collaboration

    Serve as a front door access point for Coordinated Entry (CE), including administration of VI-SPDAT or similar assessments.

    Participate in CoC case conferencing, collaborate on shared cases, and follow diversion protocols established by the Eastern PA Continuum of Care.

    Make and receive referrals through the Coordinated Entry system and advocate for appropriate housing and service matches.

    HUD Housing Counseling and Compliance

    Complete HUD Housing Counseling Certification within six months of hire, if not already certified.

    Deliver compliant housing counseling sessions on topics such as:

    Tenant rights and responsibilities

    Credit repair and budgeting

    Understanding leases and landlord communication

    Fair housing and anti-discrimination protections

    Additional Duties

    Maintain accurate, timely documentation in HMIS and case records.

    Track flexible fund usage, receipts, and budget documentation.

    Attend all required trainings, staff meetings, and funder learning circles.

    Promote culturally competent and equitable services, especially for populations at higher risk of housing instability.

    Support outreach and shelter staff with diversion opportunities and coordinated transitions when needed.

    Qualifications

    Associate’s or Bachelor’s degree in Social Work, Human Services, or related field preferred.

    At least 2 years of relevant experience, preferably in housing, homelessness prevention, or crisis services.

    Knowledge of diversion principles and Coordinated Entry systems is highly desirable.

    Strong interpersonal and problem-solving skills, with the ability to navigate difficult conversations and high-pressure situations.

    Proficient in Microsoft Office and database entry (HMIS experience preferred).

    Must have a valid PA driver’s license, reliable vehicle, and insurance.

    Able to work flexible hours, including evenings or weekends, based on client and program needs.

    Working Conditions

    Work is in office, shelter, and community-based settings, including client homes for inspections when needed.

    Must be able to lift up to 25 lbs
