PT Case Management Social Worker
We provide New Choices Waiver case management services to clients who are transitioning from living in skilled nursing facilities to living in the community. We provide ongoing support to ensure that our clients each receive the services they need to continue living as independently as possible for as long as possible. The New Choices Waiver is a Medicaid program. We provide services across the state of Utah.We are seeking a Social Worker to join our team! You will provide emotional and administrative support to clients in individual settings. Work can be done from home with 2 to 3 days out in the field.
Responsibilities:
Ongoing clinical assessments.
Case management
Individual and family support and clinical interventions
Transition clients from nursing homes to community living
Develop and execute individual treatment plans
Determine appropriate milestones to gauge client progress
Communicate client updates to all relevant parties
Facilitate referrals to other healthcare professionals and programs
Maintain accurate client documentation
Qualifications:
Previous experience in social work, counseling, or other related fields
Compassionate and caring demeanor
Ability to build rapport with clients
Strong leadership qualities
Excellent written and verbal communication skills
Reliable transportation
Current social work license, SSW or higher
