    Full-Time Hospice RN Case Manager Location: San Antonio, TX & Surrounding Areas

    Position Overview: We are seeking a Full-Time Hospice RN Case Manager to join our dedicated team. This role requires strong clinical judgment, excellent communication skills, and a heart for hospice care. The RN Case Manager is responsible for managing a caseload of patients, coordinating care with the interdisciplinary group (IDG), and providing both direct patient care and supportive guidance to families.

    Key Responsibilities:

    Manage and oversee the care of assigned hospice patients, ensuring individualized care plans are followed.

    Perform comprehensive patient assessments, symptom management, and education for patients and families.

    Attend and actively participate in Interdisciplinary Group (IDG) meetings.

    Coordinate with physicians, social workers, chaplains, and other team members to ensure seamless care delivery.

    Maintain accurate and timely documentation in accordance with hospice and regulatory requirements.

    Participate in a rotating on-call schedule, with at least one full week of on-call duties per month included in salary.

    Travel throughout San Antonio and surrounding areas to visit patients in their homes, facilities, or other care settings.

    Qualifications:

    Current RN license in the state of Texas (in good standing).

    Minimum of 1 year of hospice or related nursing experience preferred.

    Strong assessment, clinical, and problem-solving skills.

    Comfortable with travel across San Antonio and surrounding communities.

    Excellent organizational and communication skills.

    Valid driver’s license, reliable transportation, and proof of insurance.
