    About the Role

    Devoted Care Hospice is seeking a compassionate and clinically grounded Chaplain to join our interdisciplinary team. In this full-time role, you will provide non-denominational spiritual care to patients and families navigating serious illness and end-of-life, serving as a vital member of the care team alongside nurses, social workers, physicians, and aides.

    You will assess and respond to spiritual, existential, and emotional needs; facilitate rituals and vigils; and offer presence and counsel to grieving families, all while honoring the unique beliefs and values of each individual in our care.

    Key Responsibilities

    Conduct timely spiritual care assessments for newly admitted patients and document findings in the electronic health record (Kinnser)

    Develop and implement individualized plans of care addressing spiritual and existential needs in collaboration with the IDT

    Provide direct spiritual care through regular visits, listening, prayer, ritual support, and life review as appropriate to each patient's tradition and wishes

    Support families and caregivers through the dying process, at the time of death, and through bereavement follow-up

    Participate in interdisciplinary team meetings, care conferences, and family meetings

    Facilitate or coordinate community clergy, faith leaders, and volunteer spiritual care support

    Maintain accurate and timely documentation of all visits and interventions per Medicare/Medicaid and agency requirements

    Provide on-call spiritual care coverage as assigned

    Assist with staff support and team debriefs following difficult patient deaths

    Qualifications

    1–2 years of experience in hospice, palliative care, hospital, or long-term care chaplaincy preferred

    Ability to provide non-denominational, interfaith care across diverse religious, spiritual, and secular backgrounds

    Comfort with death, dying, and the full spectrum of grief responses

    Strong written communication skills and proficiency with EHR documentation

    Valid driver's license and reliable transportation required for home and facility visits
