At Hospice of Grace and Palliative Care], we believe in providing compassionate and personalized care to patients and their families during their most vulnerable times. We are dedicated to enhancing the quality of life for those facing life-limiting illnesses and are seeking a compassionate Hospice Field LVN to join our team.

Key Responsibilities:

• Provide nursing care and support to hospice patients in accordance with their individualized care plans.

• Administer medications and treatments as prescribed by the physician.

• Monitor patient conditions and report any changes to the healthcare team.

• Educate patients and families on care plans, symptom management, and resources available to them.

• Collaborate with physicians, social workers, and other healthcare professionals to ensure comprehensive patient care.

• Maintain accurate documentation of patient care and progress.

Qualifications:

• Current California LVN license.

• Previous experience in hospice, palliative care, or home health preferred.

• Strong communication and interpersonal skills, with a compassionate approach to patient care.

• Ability to work independently and as part of a team.

• Valid driver's license and reliable transportation.
