We are seeking a compassionate and dependable Hospice LPN to join our interdisciplinary team. This role is ideal for a nurse who values relationship-based care and wants to make a meaningful difference in patients’ lives.
✨ What You’ll Do
Under the supervision of a Registered Nurse, the Hospice LPN provides skilled nursing care to patients in home and facility settings in accordance with the individualized Plan of Care.
🩺 Clinical Responsibilities
Provide skilled nursing visits as assigned
Perform focused assessments and report findings to RN Case Manager
Administer medications and treatments as ordered
Monitor pain and symptom management effectiveness
Provide wound care per physician orders
Reinforce patient and family education on disease process and care plan
Assist with coordination of care alongside the interdisciplinary team
Participate in IDG meetings as appropriate
Complete timely and accurate documentation in EMR
✅ Qualifications
Active, unrestricted Virginia LPN license
Hospice or home health experience preferred
Strong assessment and communication skills
Knowledge of end-of-life care principles
Current CPR certification
Valid driver’s license and reliable transportation
Ability to travel throughout service area (mileage reimbursed)
