    Job Summary

    The Evening Shift Hospice Licensed Vocational Nurse (LVN) provides direct patient care to hospice patients during evening hours under the supervision of a Registered Nurse. This role supports continuity of care during non-daytime operations by implementing the established plan of care, managing symptoms, responding to patient needs, and collaborating with the interdisciplinary hospice team to promote comfort, dignity, and quality of life for patients and their families.

    Essential Job Functions & Responsibilities

    Provides direct patient care in accordance with the California Nurse Practice Act and hospice regulations.

    Implements nursing care based on comprehensive assessments and the patient’s individualized hospice plan of care.

    Accurately and timely documents patient visits, clinical observations, interventions, and outcomes in accordance with hospice documentation standards.

    Provides patient and family education related to diagnosis, symptom management, medications, and the established plan of care.

    Assists with pain and symptom management and evaluates patient response to treatments and medications, reporting changes promptly to the RN.

    Coordinates hospice services during the evening shift by communicating patient needs to the interdisciplinary team, including hospice aides, social workers, therapists, and on-call supervisors.

    Responds appropriately to evening clinical needs, urgent patient concerns, and changes in condition within scope of practice.

    Utilizes medical equipment and supplies effectively, safely, and efficiently.

    Participates in quality assessment and performance improvement activities as assigned.

    Maintains professional conduct and supports a calm, compassionate environment during evening hours when patients and families may require additional reassurance.

    Performs other duties as assigned by the supervising Registered Nurse or Clinical Supervisor.

    Position Qualifications

    Graduate of an accredited Licensed Vocational Nurse (LVN) or Practical Nursing program.

    Minimum of three (3) years of nursing experience; hospice, home health, community health, or medical-surgical experience preferred.

    Current, active LVN license in the State of California.

    Demonstrates knowledge of hospice philosophy and the physical, emotional, and psychosocial needs of terminally ill patients.

    Maintains current CPR certification.

    Demonstrates strong verbal, written, and organizational communication skills.

    Complies with accepted professional nursing standards and hospice practices.

    Valid driver’s license with reliable, insured transportation in accordance with state and organizational requirements.

    Ability to work independently during evening hours while maintaining appropriate communication with supervising staff.

    Physical & Work Requirements

    Ability to perform patient care activities including standing, walking, bending, lifting, and transferring patients as needed.

    Ability to travel to patient homes or facilities during evening hours.

    Ability to respond calmly and professionally to urgent or emotionally sensitive situations
