As a Hospice Liaison (Community Liaison/Representative), you’ll play a key role in increasing awareness and access to hospice services through education, outreach, and relationship-building across the local healthcare community.
This role is ideal for someone passionate about making a difference in the lives of patients and families while working collaboratively with an experienced and supportive team.
Salary and Benefits – Hospice Liaison
- Performance-based compensation structure
- $10,000 in annual salary per 1 average monthly admission (based on 3-month average length of stay)
- Example:
- Shorter LOS (1 month): ~25 admissions/month = $100,000/year
- Longer LOS (6 months): ~6–7 admissions/month = $100,000/year
- Full health, dental, and vision insurance

Key Responsibilities – Community-Based Hospice Liaison
- Monday to Friday schedule – 8:00 AM to 5:00 PM
- No weekends or on-call
- Build and maintain referral relationships with physicians, hospitals, ALFs, SNFs, other healthcare partners
- Educate referral sources and community stakeholders about hospice services, eligibility criteria, and benefits
- Conduct outreach through in-person visits, presentations, events, and networking opportunities
- Coordinate closely with internal teams to support a seamless admissions process
- Achieve or exceed monthly referral and admissions goals
- Accurately document referral activity and outcomes using CRM tools
- Stay informed on hospice industry trends, compliance regulations, and local market dynamics
- Represent the organization professionally and compassionately in all community settings
Requirements
- 1–3 years of experience in hospice, home health, or healthcare marketing
- Strong knowledge of hospice philosophy and Medicare/Medicaid guidelines
- Excellent communication, presentation, and relationship-building skills
- Ability to work independently in the field and manage a referral territory
- Valid driver’s license and reliable transportation
- Proven record of achieving referral or admissions goals
