Hospice Licensed Practical Nurse (LPN)
The Hospice LPN works under the supervision of the RN Case Manager to provide direct nursing care, emotional support, and symptom management to hospice patients.
Key Responsibilities:
· • Provide bedside care and assist with medication administration per plan of care.
· • Observe and report changes in patient condition to RN or physician.
· • Perform wound care, catheter care, and other assigned clinical tasks.
· • Maintain accurate and timely documentation.
· • Educate family members on basic patient care and comfort measures.
· • Participate in team meetings and quality improvement initiatives.
Qualifications:
· • Current LPN license (State of Georgia).
· • Minimum 1 year of nursing experience, preferably in hospice or home health.
· • Compassionate communication and team collaboration skills.
· • Valid driver’s license and reliable vehicle.
