Your Role:

• Assess and address the needs of hospice patients and their families

• Deliver nursing care according to physician orders and interdisciplinary care plans

• Collaborate closely with Nurse Case Managers and the interdisciplinary team

• Maintain clear communication with physicians, hospice staff, and external providers

• Collect and report data on patients’ physical, psychological, social, and spiritual health

What You Bring:

• Current Arizona LPN/LVN license

• Willingness to work after-hours and weekends as needed

• Strong organizational and communication skills

• Valid driver’s license, reliable vehicle, and auto insurance

• Current BLS certification

• Arizona Class 1 Fingerprint Clearance Card (or ability to obtain)

• Preferred: Hospice care experience and ability to manage emotional stress while respecting diverse lifestyles
