    Golden Oaks Hospice is seeking a compassionate bilingual Hospice Social Worker to join our care team. In this role, you will provide vital emotional support, counseling, and resource coordination to patients with terminal illnesses and their families. You will work as part of an interdisciplinary team to ensure patients receive comprehensive, personalized care that addresses their physical, emotional, spiritual, and practical needs. The ideal candidate will be bilingual in Spanish, have strong communication skills, a deep sense of empathy, and experience guiding individuals and families through the end-of-life journey.     Responsibilities

    Conduct comprehensive patient assessments to determine psychosocial needs and develop appropriate care plans.

    Provide clinical counseling and behavioral therapy to patients and families, ensuring adherence to HIPAA regulations.

    Maintain accurate medical documentation and records, including intake assessments and progress notes.

    Collaborate with interdisciplinary teams to enhance patient care and support hospice services.

    Utilize effective communication skills to engage with patients, families, and healthcare professionals.

    Qualifications

    Master’s degree in Social Work (MSW) or equivalent

    Licensed Clinical Social Worker (LCSW) or similar certification

    Strong communication and interpersonal skills

    Proficiency in medical documentation and terminology

    Experience in hospice care and patient assessment

    Knowledge of HIPAA regulations and patient confidentiality

    Skills in behavioral therapy and crisis intervention

    Familiarity with community resources and support systems

    Bilingual in Spanish
