    Position Summary

    We are seeking a skilled and compassionate Hospice Nurse Practitioner (NP) to join our interdisciplinary care team. The ideal candidate will provide high-quality palliative and end-of-life care to patients in their homes, assisted living facilities, and other care settings. This role focuses on symptom management, patient education, and collaboration with the care team to ensure comfort and dignity for our patients.

    Key Responsibilities

    Conduct comprehensive assessments and develop individualized hospice care plans.

    Provide palliative symptom management and coordinate with the hospice team for ongoing patient needs.

    Collaborate closely with physicians, nurses, social workers, and spiritual care providers.

    Educate patients, families, and caregivers on disease progression, comfort measures, and care plans.

    Document all patient encounters accurately and timely in the EMR.

    Participate in interdisciplinary team meetings, on-call rotation, and staff training as needed.

    Qualifications

    Current and unrestricted Nurse Practitioner license in [State].

    Active DEA license (or eligibility to obtain).

    Minimum of 1 year of experience in hospice, palliative care, geriatrics, or related field preferred.

    Strong clinical assessment and communication skills.

    Compassionate and patient-centered approach to care.

    Valid driver’s license and reliable transportation.
