Position Overview We are seeking a Nurse Practitioner (NP) with experience or strong interest in hospice and palliative care to provide compassionate, patient-centered medical care for individuals at the end of life.
This role blends clinical excellence with relational presence. You will support symptom management, guide complex care decisions, and collaborate closely with an interdisciplinary hospice team to ensure care aligns with each patient’s values and goals.
What You’ll Do
Perform in person visits for recertification and track benefit period schedule
Provide advanced practice care including assessment, diagnosis, and recommendations for management of pain and symptoms
Prescribe medications and treatments in alignment with hospice philosophy and patient goals as needed
Collaborate with physicians, RNs, social workers, and chaplains
Ensure compliance with hospice regulations and documentation standards

Qualifications
Active Nurse Practitioner license in Oregon
Board certification (FNP, AGNP, or related specialty)
Experience in hospice, palliative care, geriatrics, or complex care preferred
Strong communication skills and comfort with end-of-life discussions
Knowledge of hospice regulations and interdisciplinary care models
Ability to work independently while collaborating closely with a care team
