Responsibilities: • Assess patients’ conditions with a focus on pain and symptom management. • Develop individualized care plans in collaboration with the interdisciplinary team and patient’s family. • Update care plans regularly to reflect changing needs and ensure quality of life. • Monitor patient responses to treatment and adjust interventions as needed. • Coordinate closely with physicians, nurses, and other team members to provide comfort-focused care. • Offer emotional and spiritual support to patients and families throughout the end-of-life journey. • Provide care that respects each patient’s cultural and spiritual values. • Participate in team meetings to review patient care and address concerns. • Collaborate with social workers, chaplains, aides, and other support staff to ensure comprehensive care. • Communicate with outside providers to ensure continuity and support for families. • Educate patients and families about the hospice process and available services. • Accurately document care and maintain up-to-date patient records. • Adhere to all NJ hospice regulations and organizational policies. Qualifications: • Must have an active RN license in the state of New Jersey. • At least 2–3 years of clinical nursing experience required; hospice, palliative care, or related experience strongly preferred. • Deep understanding of hospice philosophy and end-of-life care principles. • Excellent communication and interpersonal skills to support patients, families, and the care team. • Shows empathy, compassion, and emotional sensitivity when working with individuals and families facing serious illness and loss. • Able to work independently while also contributing effectively as part of a team. Strong time management and organizational skills are essential. • Comfortable using electronic health records (EHR) and other healthcare software platforms. Compensation: $48 - $53 hourly
