We are seeking a compassionate and experienced Registered Nurse (RN) Hospice Case Manager to join our hospice care team in a 7 days on / 7 days off schedule. This position provides meaningful, patient-centered care to individuals and families navigating end-of-life care in the comfort of their homes.
Our chosen candidate will travel throughout our entire 60-mile service radius in this on-call role, supporting patients and families while coordinating care with the hospice interdisciplinary team.

Position Details:
• 7 days on / 7 days off schedule
• Salary: $85,000 – $89,000 annually (Possible Sign On Bonus)
• On-call hospice case management role
• Travel required within a 60-mile service radius
• Field-based hospice care in patient residences

Responsibilities:
• Administer skilled nursing care for patients of all ages in their place of residence
• Conduct patient admissions and hospice intake assessments
• Perform routine patient visits and clinical evaluations
• Coordinate care within the hospice interdisciplinary team to ensure patient and family needs are met
• Communicate with attending physicians, hospice physicians, and other providers involved in the patient’s care
• Demonstrate accountability, flexibility, respect, and integrity in patient care and team collaboration
• Submit all required documentation and paperwork within 48 hours
Qualifications:
• Current Registered Nurse (RN) license in the State of Texas
• One (1) year of hospice experience strongly preferred as an RN Case Manager or in a similar role
• Strong clinical assessment and care coordination skills
• Compassionate approach to end-of-life care
• Must have a valid Texas Driver’s License and dependable transportation
