We are seeking an experienced and compassionate Hospice RN Case Manager to join our interdisciplinary team. This role is ideal for a nurse who values autonomy, relationship-based care, and making a profound impact on patients and families.

✨ What You’ll Do

As an RN Case Manager, you will oversee and coordinate the total plan of care for a caseload of hospice patients in home and facility settings.

🩺 Clinical Leadership

Perform comprehensive patient assessments

Develop and update individualized Plans of Care

Manage symptoms and provide end-of-life education

Coordinate care with physicians and hospice Medical Director

Supervise Hospice Aides / Home Maker Aides

Ensure compliance with Medicare Conditions of Participation

Participate in IDG meetings

🤝 Care Coordination

Collaborate with social workers, chaplains, aides, and therapy services

Provide education and emotional support to patients and families

Facilitate transitions in care levels (Routine, GIP, Respite, Continuous)

Ensure timely documentation and regulatory compliance

📋 Documentation & Compliance

Complete timely and accurate EMR documentation

Maintain audit-ready clinical records

Follow agency policies and state/federal hospice regulations

✅ Qualifications

Active, unrestricted Virginia RN license

1–2 years of hospice experience preferred

Strong assessment and symptom management skills

Knowledge of Medicare hospice regulations

Current CPR certification

Valid driver’s license and reliable transportation

Excellent communication and time management skills
