About the Role:
Divine Guardian LLC is seeking a compassionate and dedicated Hospice RN to join our caring team in Tucker, GA. Responsibilities:
Provide skilled nursing assessments and direct patient care to hospice patients in home or facility settings
Develop, implement, and update individualized patient care plans in coordination with the interdisciplinary team
Manage pain and symptom control to maximize patient comfort and quality of life
Educate and support patients and family members on disease progression, medications, and end-of-life care
Coordinate with physicians, social workers, chaplains, and aides to ensure holistic patient care
Accurately document patient assessments, care plans, and clinical notes in compliance with hospice regulations
Requirements:
Active RN license in the state of Georgia (required)
Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
Strong knowledge of pain management, symptom control, and end-of-life care protocols
Excellent communication and interpersonal skills with patients, families, and care teams
Ability to work independently, manage a patient caseload, and prioritize effectively
Valid driver's license and reliable transportation for patient visits in the Tucker, GA area
