We are currently seeking an experienced and empathetic Registered Nurse (RN) to join our interdisciplinary team. Position Summary
The Hospice RN is responsible for leading, coordinating, and delivering comprehensive care aligned with the patient’s plan of care. You will serve as a key member of our care team, supporting patients and families physically, emotionally, and spiritually throughout their journey.
Key Responsibilities
Conduct initial and ongoing assessments for hospice patients
Develop and maintain individualized care plans
Collaborate with physicians, hospice aides, and interdisciplinary team members (IDG)
Provide direct patient care and educate family members on care and the dying process
Monitor and evaluate patient responses to care plans and adjust as needed
Maintain accurate, timely, and compliant clinical documentation
Participate in IDG case conferences, QAPI meetings, and staff development sessions
Perform on-call duties, including nights, weekends, and holidays
Supervise Hospice Aides and LPNs, including monthly evaluations
Provide bedside care when hospice aide is unavailable
Required Qualifications
Graduate of an accredited School of Nursing
Active Registered Nurse License in the State of Virginia
Minimum 1 year of patient care experience ( preferred) / New Grads welcome to apply.
Valid Driver’s License and up-to-date Auto Insurance
Strong communication, documentation, and problem-solving skills
Knowledge of hospice regulations (Medicare/Medicaid) and end-of-life care standards
Ability to lift and move up to 250 lbs safely (with assistance) and travel to patient homes
