About the Role:
Graceful Legacy Hospice in Fredericksburg, VA is looking for a compassionate and dedicated Hospice RN to join our care team. Responsibilities:
Conduct comprehensive patient assessments and develop individualized plans of care for hospice patients
Manage pain and symptom control to maximize patient comfort and quality of life
Coordinate care with interdisciplinary team members including physicians, social workers, and chaplains
Provide education and emotional support to patients and families regarding the hospice process
Perform skilled nursing visits in patient homes, assisted living facilities, or care settings throughout the Fredericksburg, VA area
Accurately document patient care, medication administration, and clinical notes in compliance with hospice regulations
Respond to after-hours calls and provide on-call nursing support as needed
Requirements:
Active RN license in the state of Virginia
Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
Strong knowledge of pain management, symptom control, and end-of-life care best practices
Excellent communication and interpersonal skills with patients, families, and care teams
Ability to work independently and manage a patient caseload in a field-based environment
Valid driver's license and reliable transportation for home visits in the Fredericksburg, VA area
Compassionate, patient-centered approach with emotional resilience in sensitive care situations
