We are dedicated to providing compassionate, high-quality end-of-life care throughout San Antonio and surrounding communities. One full week of on-call coverage required and included in salary
Responsibilities
Provide skilled nursing care to hospice patients in homes, facilities, assisted living communities, and other care settings in accordance with the plan of care
Perform routine visits, admissions, recertifications, PRN visits, and death visits as needed
Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team
Administer medications and treatments as ordered while ensuring patient comfort and safety
Monitor changes in condition and communicate updates to physicians and team members promptly
Educate patients and families regarding hospice services, symptom management, coping strategies, and end-of-life care options
Support the emotional and spiritual needs of patients and families during this critical time
Complete all clinical documentation accurately and within 24 hours of each visit
Participate in biweekly IDG (Interdisciplinary Group) meetings
Maintain compliance with Medicare, state regulations, and company policies
Provide after-hours support during assigned on-call rotation
Qualifications
Current and active Texas Registered Nurse license in good standing
Minimum 2 years of nursing experience required
Hospice, home health, palliative care, or end-of-life care experience preferred
Strong clinical assessment, time management, and critical thinking skills
Excellent communication and interpersonal skills
Ability to work independently while collaborating effectively with a team
Compassionate, patient-centered approach to care
Current BLS/CPR certification required
Reliable transportation, valid driver’s license, and current auto insurance required
Experience with EMR systems preferred (Axxess experience a plus)
