We are looking for a dedicated Weekend On-Call Hospice RN to join our team and help provide exceptional patient care.

Position Summary The Weekend On-Call Hospice RN will be responsible for triaging patient needs, making home visits as necessary, and providing skilled nursing care during after-hours and weekend shifts. This role ensures continuity of care and timely response to urgent patient needs.

Key Responsibilities

Respond to patient and family calls during assigned on-call hours.

Assess patient condition and provide appropriate interventions.

Conduct home visits as needed for symptom management, medication administration, and urgent care needs.

Collaborate with the interdisciplinary team to coordinate ongoing patient care.

Document all visits and communications in accordance with agency, state, and federal regulations.

Provide emotional support and education to patients and families.

Qualifications

Current Texas RN license in good standing.

Minimum one year of hospice, home health, or related nursing experience (preferred).

Strong clinical assessment and critical thinking skills.

Compassionate and empathetic approach to patient care.

Ability to work independently and manage time effectively.

Reliable transportation and current auto insurance.

Schedule & Compensation

Weekend On-Call: Typically Friday 5:00 PM – Monday 8:00 AM.
