Hospice RN Case Manager – Hybrid | Richmond, VA

We are seeking an experienced Hospice Registered Nurse Case Manager (RN) to join a compassionate and mission-driven hospice team serving patients throughout the Richmond, Virginia area. This role combines autonomy, meaningful patient relationships, and strong interdisciplinary support while allowing nurses to make a real difference in patients’ lives.
This position requires prior hospice RN experience with a hospice agency or program.
A Day in the Life
Begin your day visiting patients in their homes, focusing on comfort, dignity, and quality of life
Assess patient needs, manage symptoms, and adjust care plans as conditions change
Collaborate closely with physicians, hospice aides, LPNs, social workers, and chaplains
Support and educate families, helping them feel confident in the care their loved ones receive
Document patient care efficiently using Consolo EMR
Advocate for your patients while maintaining autonomy in how you structure your day
Key Responsibilities
Serve as the primary RN coordinating care for assigned hospice patients
Develop, implement, and update individualized plans of care
Determine visit frequency based on patient acuity and family needs
Provide guidance and oversight to Hospice Aides and LPNs
Communicate patient condition changes to physicians and the interdisciplinary care team
Participate in on-call rotation
Qualifications
Active Registered Nurse (RN) license – Virginia
Graduate of an accredited nursing program (ADN or BSN)
Minimum 2 years of RN hospice experience with a hospice agency/program
Strong clinical judgment, compassion, and communication skills
Reliable transportation, valid driver’s license, auto insurance
CPR certification
