Hospice Registered Nurse (RN)
The Hospice RN provides skilled nursing care to terminally ill patients and their families in homes, facilities, or inpatient settings. The RN coordinates all aspects of care, focusing on comfort, dignity, and quality of life.
Key Responsibilities:
· • Conduct comprehensive patient assessments and develop individualized care plans.
· • Administer medications and treatments for pain and symptom management.
· • Educate patients and families on disease process, care techniques, and expectations.
· • Collaborate with physicians, social workers, chaplains, and volunteers.
· • Document all care accurately in EMR according to regulatory standards.
· • Participate in interdisciplinary team (IDT) meetings and case conferences.
· • Provide emotional support and crisis intervention for patients and families.
· • Ensure compliance with Medicare, state, and agency policies.
Qualifications:
· • Current RN license (State of Georgia).
· • Minimum 1 year of clinical nursing experience (hospice or home health preferred).
· • Excellent communication, assessment, and critical thinking skills.
· • Valid driver’s license and reliable transportation.
