About the Role:

Responsibilities:

Conduct comprehensive assessments to identify patient and family needs.

Provide emotional support and counseling to patients and their families.

Develop and implement individualized care plans in collaboration with interdisciplinary teams.

Facilitate communication between patients, families, and healthcare providers.

Assist with accessing community resources and support services.

Document patient interactions and care plans in compliance with regulations.

Participate in team meetings and contribute to quality improvement initiatives.

Maintain confidentiality and adhere to ethical standards in all interactions.

Requirements:

Master's degree in Social Work (MSW) from an accredited program.

Current state licensure as a Licensed Clinical Social Worker (LCSW).

Minimum of 2 years of experience in hospice or palliative care settings.

Strong interpersonal and communication skills with a compassionate demeanor.

Ability to work collaboratively in a multidisciplinary team environment.

Proficient in case management and crisis intervention techniques.

Knowledge of community resources and support services for patients.

Flexibility to manage a varied caseload and adapt to changing needs.
