Responsibilities: • Meet with patients and families in their homes, facilities, or by phone to understand their needs and concerns • Provide emotional support, counseling, and crisis intervention during difficult moments • Connect patients and families with community resources, benefits, and support services • Advocate for each patient’s wishes, values, and cultural traditions in the care plan • Work closely with nurses, aides, physicians, and chaplains to create a truly collaborative care experience • Help families navigate advance care planning, paperwork, and decision-making • Offer grief and bereavement support to patients and loved ones • Maintain clear, timely documentation to ensure quality care and regulatory compliance • Participate in team meetings to share updates, coordinate care, and problem-solve challenges Qualifications: • Active New Jersey license (LSW or LCSW) • Experience in hospice, palliative care, or healthcare preferred • Knowledge of end-of-life care and family support needs • Strong communication and interpersonal skills • Ability to build trust and provide guidance during sensitive situations • Familiarity with local resources and community services Compensation: $70,000 - $85,000 yearly
