    Gateway Unlimited Living LLC is a Minnesota-licensed 245D service provider dedicated to helping individuals achieve greater independence, stability, and quality of life in their communities.

    We are seeking a Housing Case Manager to join our team and provide Moving Home Minnesota (MHM) services to individuals transitioning to and maintaining housing in the community.

    This position is ideal for someone who is compassionate, organized, resourceful, and passionate about helping people find stable housing and successfully establish themselves in the community.

    Position Summary

    The Housing Case Manager will work directly with individuals to provide individualized housing-related support through the Moving Home Minnesota program.

    The Housing Case Manager will assist individuals with identifying appropriate housing, navigating the housing process, coordinating community resources, preparing for a move, and developing the skills and supports necessary to successfully maintain their home and community living arrangement.

    This position requires strong communication skills, excellent organization, independent decision-making, and the ability to build positive relationships with individuals, landlords, community agencies, service providers, and other professionals.

    Essential Responsibilities

    Housing Navigation & Support

    Meet individually with participants to identify housing needs, preferences, and goals.

    Assist individuals in identifying safe, affordable, and appropriate housing options.

    Help individuals understand housing applications, leases, tenant responsibilities, and housing expectations.

    Assist with completing housing applications and gathering required documentation.

    Help individuals communicate with landlords, property managers, and housing providers.

    Assist with identifying available community housing resources.

    Help individuals overcome barriers to obtaining and maintaining housing.

    Provide education regarding tenant rights, responsibilities, and community living.

    Support individuals in developing skills necessary to successfully maintain their housing.

    Moving Home Minnesota Services

    Provide person-centered Moving Home Minnesota services based on each individual's needs and service plan.

    Assist individuals transitioning from institutional or other settings into community-based housing when applicable.

    Coordinate with the individual's support team throughout the housing transition process.

    Assist with planning and preparing for a successful move.

    Help coordinate community resources and services needed to support a successful transition.

    Assist individuals with establishing connections to community resources.

    Monitor progress toward housing and community-living goals.

    Identify concerns or barriers and assist in developing solutions.

    Community Resource Coordination

    Develop and maintain knowledge of local housing and community resources.

    Connect individuals with appropriate community programs and resources.

    Collaborate with county case managers, service providers, landlords, housing organizations, and other professionals.

    Advocate for individuals while maintaining appropriate professional boundaries.

    Assist individuals in accessing resources that promote stability and independence.

    Documentation & Compliance

    Complete accurate and timely service documentation.

    Maintain records according to company policies and applicable Minnesota requirements.

    Document progress toward individual goals.

    Maintain confidentiality of participant information.

    Follow individual service plans and program requirements.

    Report significant changes, concerns, incidents, or barriers appropriately.

    Participate in team meetings, supervision, and required training.

    Maintain all required training and qualifications.

    Qualifications

    The ideal candidate will have:

    Experience in housing, case management, social services, disability services, human services, or a related field.

    Strong organizational and time-management skills.

    Excellent written and verbal communication skills.

    Ability to work independently and manage a flexible schedule.

    Strong problem-solving and advocacy skills.

    Ability to build professional relationships with individuals, families, landlords, counties, and community partners.

    Ability to maintain professional boundaries and confidentiality.

    Proficiency with computers, email, electronic documentation, and Microsoft Office or similar programs.

    Valid driver's license and reliable transportation.

    Ability to travel throughout the assigned service area.

    Education or experience in social work, human services, housing, disability services, or a related field is preferred.

    Travel Requirements

    Travel is an essential part of this position.

    The Housing Case Manager will regularly travel to:

    Participants' homes

    Housing properties

    Community locations

    County and community partner offices

    Meetings with landlords and housing providers

    Other locations necessary to provide Moving Home Minnesota services

    The ability to independently manage a community-based schedule and travel between appointments is required.
