    Housing El Dorado is a dynamic, new non-profit on the Western Slope of El Dorado County committed to increasing affordable housing and services for our homeless neighbors. The Outreach Worker handles case files from a variety of HED Programs; We need a self-starter who is flexible to meet a variety of needs and will work to prioritize and complete the following kinds of tasks:

    Travel through neighborhoods and remote areas where people experiencing homelessness are believed to reside in order to identify and make contact with Clients

    Build rapport and develop relationships with Clients experiencing homelessness

    Learn about and document the needs, habits, and demographics of Clients

    Encourage Clients to take advantage of housing programs, shelter, and related services through motivational interviewing and other forms of trauma-informed care

    Help clients enroll in mainstream benefits such as health insurance and SSI/SSDI

    Help clients understand the relevant options in their search for housing or shelter

    Communicate consistently with other service providers, and arrange for “warm handoffs” to providers that have relevant services

    Arrange appointment scheduling, and transportation to scheduled appointments

    Distribute emergency supplies: clothing, toiletries, and communication equipment

    Regularly communicate with HED Case Manager, Coordinated Entry (CE), and other staff regarding participant needs

    Ask clients to sign release of information forms, and with their permission, collect and report program data in electronic databases such as HMIS

    Qualifications

    Excellent verbal and written communication, including cultural competency skills

    Willingness & ability to safely travel to and through a wide variety of neighborhoods and remote areas in our region

    Demonstrated ability to work with diverse populations, including persons experiencing homelessness

    Strong documentation skills, including maintenance of paper files and online data

    Familiarity with and commitment to the principles and practices of Housing First, trauma-informed and participant-centered care, harm reduction, motivational interviewing, and safeguarding participant confidentiality.

    Experience working on a team and being collaborative

    Knowledge of community resources for households with very low income

    Salary

    $20/hour, up to 20 hours per week
