RESPONSIBILITIES

Human Touch Home Health Care is seeking a full-time RN Case Manager. An RN Case Manager plans, organizes, and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The RN builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

REQUIREMENTS

• Must have an active RN license and CPR certificate
• Must have a current driver's license and auto insurance.
• Ideal applicants will be reliable, organized.
• Must have dependable transportation
• All applicants must pass a drug test.

Job Type: Full-time

Salary: $1,800.00 to $2,200.00 /week
