The Position
As a Clinic Triage LPN , you'll serve as a first-line clinical resource by assessing and triaging patient concerns via telephone and electronic messages. Working in close collaboration with registered nurses and providers, you'll use protocols and clinical judgment to guide patient care, education, and follow-up in a remote setting.
Your Day-to-Day Work
- Triage incoming patient communications and nurse notes to assess urgency and next steps.
- Provide patient education and care advice using clinical guidelines and collaboration with RNs and providers.
- Support medication-related education and coordination of refills or clarifications.
- Review abnormal results and coordinate follow-up under RN/provider direction.
- Ensure timely, accurate documentation in the electronic medical record (EMR).
- Refer patients to appropriate providers or resources based on assessment.
- Serve as a liaison between providers, patients, families, and external care partners.
- Assist with care plan implementation, policy feedback, and remote care optimization.
Environment and Shift Details
This is a full-time remote position. Candidates must complete initial onsite training in Washington and may have intermittent onsite responsibilities. All remote work must meet CSC's telecommuting standards, with dedicated focus during scheduled hours. Occasional travel to local clinics may be required.
Experience & Qualifications
Must-Haves
- Graduate of an accredited nursing program.
- Active Licensed Practical Nurse (LPN) license in the State of Washington.
- Current BLS certification.
- Excellent communication, organization, and patient service skills.
- Ability to triage based on clinical priorities and patient safety.
- Comfortable with technology and electronic health record systems.
Nice-to-Haves
- 3+ years of telephonic triage or cardiovascular care experience.
- Experience working in Epic or other EMR systems.
- Knowledge of cardiac conditions and ambulatory nursing workflows.
