    Summary:

    The Case Manager is a healthcare professional who serves as patient advocates to support, guide and coordinate care for patients, families, and caregivers as they navigate their health and wellness journeys.

    Duties/Responsibilities:

    Engages with the patient and assists with identifying resources to enhance treatment; works collaboratively with Behavioral Health Director to assist with obtaining necessary outside support for patient.

    Reviewing records and applications; conducting orientations

    Determines patients’ requirements by completing intake interviews; determining need for medical, psycho-social, and psychiatric evaluations; reviewing therapist evaluations, treatment objectives, and plans.

    Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation.

    Monitors cases by verifying patients’ attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.

    Maintains patient records by reviewing case notes, logging events and progress.

    Communicates patient’s progress by conducting interdisciplinary meetings and evaluations; disseminating results and obstacles to team; identifying treatment influences.

    Enhances department and organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.

    Assist in day to day needs for Drug Court such as but not limited to scheduling group attendance, keeping documentation for patient attendance and drug screen results, and coordinating appointments.

    Collaborates with the Behavioral Health Director and other appropriate parties.

    Qualifications:

    Bachelor's degree from an accredited college or university in social work, social services, human services, or a related field.

    Two years of professional experience providing case management to children and families preferred in a rural environment.

    Professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors highly preferred.
